Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our responsibilities

Sunland ADHC is required by law to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your health information

Treatment. To provide and coordinate your care, including with your physician, therapists and your Medi-Cal managed-care plan.
Payment. To obtain authorization and payment from Medi-Cal, your health plan or other payers.
Health care operations. For quality improvement, licensing surveys, training and administration.
Family and caregivers. With your agreement, or when you are unable to agree and it is in your best interest, we may share relevant information with family members or others involved in your care.
As required by law. Including public health reporting, reports of suspected abuse or neglect of an elder or dependent adult, and responses to lawful requests.
Emergencies. To emergency responders and hospitals when needed.

Other uses and disclosures, including most uses for marketing and any sale of PHI, require your written authorization, which you may revoke at any time in writing.

Your rights

To inspect and receive a copy of your health record
To request corrections
To request restrictions on certain uses and disclosures
To request confidential communications (for example, calls to a specific number)
To receive an accounting of certain disclosures
To receive a paper copy of this notice, and a copy in your preferred language
To be notified if a breach of your unsecured PHI occurs

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at (818) 570-7771 or by mail to 8701 Sunland Blvd, Sun Valley, CA 91352, or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

Changes to this notice

We may change this notice and make the new notice apply to information we already hold. The current notice is posted at the center and on this website.
