Eligibility and cost

Adult Day Health Care (ADHC) is covered by Medi-Cal through a benefit called Community-Based Adult Services (CBAS). For eligible members the program, meals and transportation cost $0.

Three quick questions

  1. Does your loved one have Medi-Cal (including Medi-Cal through a managed-care plan)?
  2. Are they 18 or older with a health condition that affects daily life, such as memory loss, a chronic illness, a disability or recovery after a hospital stay?
  3. Do they live in or near Sun Valley, Sunland, Tujunga, Pacoima, North Hollywood or Burbank?

What CBAS is, in plain English

It is a Medi-Cal benefit

CBAS pays for Adult Day Health Care for people who need help to keep living safely at home.

The health plan approves it

Your Medi-Cal managed-care plan does a short face-to-face assessment, usually at home or at the center, and authorizes the days per week.

We handle the paperwork

Sunland gathers the physician form, submits the request and follows up with the plan so you do not have to.

There is no cost to the family

Program, meals and transportation are covered. No copays, no bills.

How enrollment works

  1. Call or book a tour

    Tell us about your loved one. We answer questions in your language and schedule a visit.

  2. We check the plan and gather the physician form

    You give us the health-plan card; we contact the doctor for a brief health information form.

  3. Health-plan assessment

    A nurse from the plan meets your loved one, usually within a few weeks, and authorizes the program.

  4. Start date and transportation schedule

    We set the first day, confirm pickup times and welcome your family to Sunland.
