Open roles
Registered Nurse (RN) or LVN
Daily health monitoring, medication oversight and care planning with the program director. Part-time or full-time [TBD].
Activity Coordinator
Plans and leads the daily program: exercise, cognitive games, music, arts and lectures.
Program Aide
Supports participants during activities and meals, assists with personal care as needed.
Driver
Door-to-door transportation in our vans. Clean driving record required; wheelchair-lift experience a plus.
Social Worker (MSW/LCSW)
Assessments, care plans, family support and coordination with health plans [TBD].
